Second-Generation Endocuff Again Found to Be Effective in New Randomized Controlled Trial

The second-generation Endocuff (Olympus America, Center Valley, Penn, USA) is called Endocuff Vision (ECV). It differs from the original Endocuff by having only a single row of fingers, which are longer than the fingers in the original version. There has been some concern about whether ECV is as effective as the original Endocuff. 

In a randomized controlled trial performed in 9 private practices in Berlin and Hamburg, Germany, 1416 patients undergoing screening colonoscopy were randomized to ECV or standard colonoscopy. The ECV group had an adenoma detection rate (ADR) of 39.5% versus the standard colonoscopy group which had an ADR of 32.2% (a 7.3% increase; P=.004). Detection of adenomas <10 mm in size increased by 9.3% with ECV. Adenomas per colonoscopy also increased from 0.51 to 0.57 (P=.045). Withdrawal time was 7 minutes in each group, excluding biopsy and polypectomy.

COMMENT

This study, the first to evaluate ECV in a pure screening population, provides important reassurance about the efficacy of ECV, or second-generation Endocuff.
