Increased Risk of Eosinophilic Gastritis and Enteritis in Families of Patients With Eosinophilic Esophagitis

Eosinophilic esophagitis (EoE) is influenced by both genetic and environmental factors. The genetic risk of distal eosinophilic gastrointestinal diseases in patients with EoE is not known. This study examined the risk of eosinophilic gastritis/gastroenteritis (EG/EGE) and eosinophilic colitis (EC) in patients with EoE and their first-, second-, and third-degree relatives.
This study consisted of 8455 patients with EoE, 396 with EG/EGE, and 172 with EC, identified from the Utah Population Database. There was an increased risk of EG/EGE for first-degree relatives (relative risk [RR], 7.77) and second-degree relatives (odds ratio, 1.94; 95% confidence interval, 1.35-2.70) of the EoE probands. There also was an increased risk of EG/EGE shown in EC probands and their first-degree (RR, 3.21) and third-degree relatives (RR, 1.81). EG/EGE probands without EoE were shown to be at high risk for EC (RR, 2026.83), and EC probands without EoE were found to have an increased risk for EG/EGE (RR, 2026.83).
COMMENTPatients with EoE and their first- and second-degree relatives have an increased risk of EG/EGE, suggesting shared genetic factors among these disorders. Providers should be encouraged to collect a detailed family history of eosinophilic gastrointestinal diseases when symptoms are present.
